some of the best teams from across Wisconsin and the Midwest at Reddan Soccer Park.

| Game Information |
|
Each team will be guaranteed a minimum of three (3) games. U-10 through U-12 teams will play 25
minute halves, U-13 through U-15 teams will play 30 minute halves, and U-16 through U-19 will play 35
minute halves. Please note: local teams, within 100+ miles of Dane County may play their opening
games on Friday evening. A three-person refereeing system will be used in all U-11 through U19
games and a one-person refereeing system in all U10 games.

| Awards |
|
Individual awards/trophies will be given to first and second place winners in the U-11 to U-19 age
divisions. All U-10 participants will receive a participation award.

Eligibility and Acceptance
Eligibility and Selection for U13 through U-19:
All players and teams must be USYSA-registered for the 2008-2009 season. Only official USYSA
league-rostered players are allowed. Rosters are limited to 18 players. Three (3) guest players
are allowed. Teams will be selected on the basis of past competitive records, geographic diversity
and date of application. It is essential that you include all information that is requested on
the online application. You must also remember to block out the tournament dates on your League
application!! The tournament will not modify game times to accommodate teams that fail to do this.
Eligibility and Selection for U-9 through U12:
All U-10 through U12 players and teams must be USYSA-registered for the 2008-2009 season. Only official
league-rostered players are allowed. U10 rosters are limited to 12 players. All games will be 6v6. Teams
will be selected on the basis of date of application. U-11 and U-12 rosters are limited to 14 players,
three (3) guest players are allowed. All games will be 8v8. Teams will be selected on the basis of past
competitive records, geographic diversity and date of application. It is essential that you include all
information that is requested on the application.
Acceptance Notification:
Team acceptance e-mails will go out by August 8th. There will be no refunds after a team is accepted for
participation. Teams not accepted will have their entry fee refunded with a letter of notification.

- A copy must be left with Tournament officials at registration and will not be returned to you.
- Guest Player Form and Instructions
- Download USYSA Guest Player Form (see forms section).
- A copy must be left with Tournament officials at registration.
- Due at Team Registration/Check-in
- State association approved "Application to Travel" form required for non-Wisconsin Teams only (see forms section).
- Copy of State Association approved Team Roster & Guest player form.
- Current USYSA passes for all U11 - U19 players.
- Copies of Wisconsin Release of Liability and Consent for Medical Treatment forms for all players (see forms section).
- Emergency contact information for manager and/or coach while attending the tournament - captured on 56er Roster form, please note hotel phone number needed.
